Welsh Government wrote the Plan for Independent Living in 2013.
The plan is about what they are doing to make Wales a better place for disabled people.
We want to make sure everyone has the same chances and can live their lives how they want.
Welsh Government have made a survey to find out what disabled people need.
This survey will help make sure they know what to do next.
