Launching a new Digital Health Assessment Platform

The Value-Based Healthcare team at CTM is excited to announce a major initiative to enhance patient outcomes and experiences across the health board. In our continuous effort to provide exceptional care, we are introducing a new digital health assessment platform to transform the collection of patient-reported measures throughout the patient health journey.

We are rolling out an innovative digital Patient Reported Outcome Measures (PROMs) collection platform, set to launch in the summer of 2024. This new platform will reshape how we collect and utilise patient-reported data, enhancing the overall quality of care and ensuring that patients benefit from more efficient and effective health assessments while keeping patient-centred care at the forefront of everything we do.

Initially, we will focus on collecting data from the Heart Failure and Lymphoedema services to gather valuable insights and learnings that will inform the broader implementation of the platform. These areas will provide crucial real-world experiences, allowing us to refine the platform and ensure optimal patient outcomes.

Patients are encouraged to help shape the future of healthcare by sharing their needs and health status with their care team once the new process is launched.

We are also actively encouraging all staff to participate in this initiative and explore how Patient Reported Outcome Measures (PROMs) are currently being used, and how they could be integrated into their services in the future. Staff involvement is crucial to the successful implementation of this new platform. To gain a deeper understanding of PROMs and Value-Based Health Care, staff are invited to complete an online course, provided by Swansea University.
