Independent External Review of Homicides published today
An evaluation of homicide reviews undertaken by Healthcare Inspectorate wales since 2007.
Since 1 January 2007, we have been responsible for conducting independent external reviews when a homicide has been committed by individuals known to mental health services in Wales.
The Welsh Government is responsible for deciding which reviews we undertake and each review is decided on a case by case basis.
Where reviews include elements that relate to social services, we include Inspectors from Care and Social Services Inspectorate Wales (CSSIW) in the review team.
We have undertaken 13 external reviews since 2007. Whilst the circumstances and details of each case have been different, common themes and findings are apparent. This evaluation review draws these themes together and assesses the impact that each review has had on services that are being provided to those who use mental health services.
