Healthcare Inspectorate Wales are looking at how health services communicate with each other when a person is discharged from hospital back to the care of their GP
Hospitals should make sure that the information they send back to a GP (when a person is discharged from hospital) is as helpful as possible so that a there is no disruption to the care that they may need. During our inspections we have found that this isn’t always the case.
We are now undertaking an all Wales review that will focus on the communication between hospitals and practices to check the quality of the documentation used to support patient discharge.
