Hospitals should make sure that the information they send back to a GP (when a person is discharged from hospital) is as helpful as possible so that a there is no disruption to the care that they may need. During our inspections we have found that this isn’t always the case.
We are now undertaking an all Wales review that will focus on the communication between hospitals and practices to check the quality of the documentation used to support patient discharge.

If you’ve been discharged from in the last two years you can help us with this work by telling us about your experiences when you were discharged from hospital. Please complete our online survey providing as much information as possible. The results from this survey will help us understand any issues people in Wales may be experiencing.
